The state day participation form and the tournament applications can now be viewed, filled out online, and printed in a consistent format independent of your printer type. To view any of the application forms click on the tournament name. The forms will be displayed in PDF format using Adobe Acrobat Reader 5.1 or higher. Please allow a couple of minutes for the form to download. After you have filled out the form, you can print it by clicking on the print button on the Acrobat Reader menu bar NOT the print button on the internet browser menu bar. If you have problems, check the Trouble Shooting tips below.

Trouble Shooting Viewing, Opening or Printing the Entry Forms
  1. You must have Adobe Acrobat version 5.1 or higher to view or open the forms. Ealier versions will not open the forms. Click on the 'Get Acrobat Reader' link above to download the latest version (5.1) free.


  2. If you have the latest version of Acrobat Reader and the forms still won't open, try rebooting your computer. If your computer has been on for any length of time, or your have been opening and closing several applications prior to trying the access the forms, then your computer probably doesn't have enough free memory to open the application. Rebooting your computer or shutting it down for a minute and then restarting it should take care of the situation. Be sure to give Acrobat Reader time to load - it may take a minute or two.


  3. To print the forms, you must use the printer icon on the Acrobat Reader menu bar. If you try to print it out using the print command, or printer icon on your web browser menu bar, all you get is a blank page.
