New or Renewal by 3/15/2003- $25.00
Renewal after 3/15/2003 - $35.00

Make check Payable to VSWGA for $25.00 (new or renewal by March 15, 2003)
or $35.00 (renewal after March 15) and mail with application to:

Eligibility Requirements:

  1. You must be a dues paying member of a Vermont golf course that sponsors a VSWGA state day.
  2. You must have a Ghin index of 36.0 or less.

Note: A player whose index is above a 36.0 prior to March 15 and does not qualify at that time can become eligible once her handicap is low enough and would be assessed the original $25 fee.

All others with qualifying indices prior to March 15 must submit payment on or before that date to avoid a late fee.

If you know of anyone who does not receive this application and would like one, please make a copy for them or refer them to this website (vswga.org), or have them contact Shari Pfannenstein at 802-773-4864 for more information.

Fill in the information above (please print clearly) and include a check for $25.00 before March 15 or $35 after March 15 made out to VSWGA. Mail the form and the check to:
